Blog: Home Care Backed By Health Checks

In his latest blog, Bluebird Care Calderdale’s Shabir Hussain looks at a new health check service that is being introduced to help support the organisation’s Home Care clients.

At Bluebird Calderdale, all our clients are now able to benefit from a new health check called “Bluebird Care Assist”.

Bluebird Care Assist is based on a universally recognised system, used across the NHS, to provide a standardised approach to monitoring vital signs, improving patient safety, and supporting clinical decision-making. It also ensures consistency in care, helping healthcare professionals respond swiftly to changes in a patient’s condition.

The Bluebird Care Assist health and wellbeing check will be carried out at regular Assessment and Care Reviews, which usually take place with clients every 6 months.

During the checks that make up Bluebird Care Assist, a number of vital sign readings will taken along alongside a general well-being questionnaire.

The vital signs that are to be recorded during the checks are:

  • Blood pressure and pulse
  • Temperature
  • Breathing and oxygen saturation
  • Alertness
  • General wellbeing status assessed via the questionnaire

These checks offer the potential to identify a decline in a customer’s physical and/or mental/emotional wellbeing.  A report is generated after every Bluebird Care Assist visit to a client, enabling prompt action to be taken if necessary.
