Patient Survey for Patients with long term conditions

Calderdale and Huddersfield NHS Foundation Trust, which runs Huddersfield Royal Infirmary and Calderdale Royal Hospital would like to hear from people with  long term conditions with regard to hospital outpatients:

People with long term conditions often have regular follow-up appointments to return to hospital outpatient clinics for their ongoing care. For some patients these visits to hospital may not always be required and they would benefit more from being able to access appointments or support at a time when they have a specific clinical need or concern. ‘
Patient initiated follow ups’ (PIFU) have been designed to support this approach – allowing patients to access clinical teams when they feel they need to get some help with their condition or for example have a flare up of symptoms. Moving on to a PIFU pathway will always be a mutual agreement between the patient and the clinician.
Calderdale and Huddersfield NHS Foundation Trust would really value your feedback on what your thoughts are on the introduction of this way of offering appointments. This survey will take approx. 5 minutes to complete. All responses will be treated in confidence.
